DESCRIPTION:Join us for an insightful Expert Webinar focused on how the Carlton-Cook-Lake-St. Louis County Community Health Board utilizes Social Determinants of Health (SDOH) data from their electronic health record (EHR) system to support community initiatives\, pursue grant funding opportunities\, and engage policymakers.
