Join us for an insightful Expert Webinar focused on how the Carlton-Cook-Lake-St. Louis County Community Health Board utilizes Social Determinants of Health (SDOH) data from their electronic health record (EHR) system to support community initiatives, pursue grant funding opportunities, and engage policymakers. During this session, attendees will learn how the organization uses pre-formatted SDOH reporting data related to food insecurity, housing instability, transportation barriers, and other crisis indicators to better understand community needs and strengthen funding applications.
The webinar will demonstrate how data extracted from the EHR can be combined with state-level public health data to identify trends, support grant funding efforts, and provide meaningful insights to policymakers and community stakeholders. Participants will gain practical examples of how the Community Health Board leverages this information to advocate for resources and improve outcomes for vulnerable populations throughout Northeastern Minnesota.
